Urgent Care/After-hours Info
Our after-hours urgent care clinic is open every
Monday to Thursday from 5 pm - 8 pm and on
Saturdays 8 am - 11 am for urgent medical issues.
Please call ahead to book an appointment time. This is not a walk-in clinic.
After 5 pm and on weekends, you may also call the Telephone Health Advisory Service at 1 (866) 553-7205 to speak to a Registered Nurse.

Sick Note for Work or School - $25.00
Prescription Note for Massage or Orthotics - $25.00
Health Physical Form for School or Camp - $30.00 -$50.00
Drivers Medical - $130.00
Disability Tax Credit Form - $120.00
Prior Authorization Medication Form - $50.00
Medical Certificate for Sick Benefits - $25.00 per page
Short Term Disability/Fitness for Work form - $25.00 per page
Other forms - $25.00 per page
Rush fee for forms (Physician’s discretion) $30.00
Per CPSO guidelines, physicians have 45 days to complete and return forms. Please allow adequate time for processing. Rush requests may incur additional fees.
Cryotherapy (plantar warts exempt) - $30.00 up to 3 lesions, additional $10.00 for each beyond first 3.
Skin tag removal - $50.00
TB testing for work or travel - $40.00 per step
Ear Flush - $40.00
Pregnancy test - $10.00
Records Transfer by USB - $35.00
Printed Records - $30.00 up to 20 pages, additional 0.25 per page beyond first 20

Patient Code of Conduct
We come to work every day to care for our patients. As a patient, you are expected to treat our administrative staff, your provider, and all other practice staff courteously- without violence, abuse, harassment, disruptive, or inappropriate behaviour. All staff have the right to a safe work environment as per provincial legislation (Bill 168) Policies.
